ROJOSON’s Inputs (August 21, 2012):

QI-BD1: Who can be called or considered “Breast Specialists” or a breast health or wellness specialist?

General Concept:

Someone, a person, who is constantly interested and devoting a big portion of his or her time, lifetime, weekly if not daily, in studying and researching to develop and refine his  / her expertise, in terms of knowledge and skills, in the management of certain, if not all issues, related to breast health or wellness.

Qualifiers in the general concept

“Certain, if not all issues related to breast health”

With medicine being an inexact science and with the management of breast health or wellness being  multi-faceted, no person on earth will be able to know, more so to become an expert, in everything regarding breast health or wellness issues and in the management of patients with a breast health or wellness problem.  Because of this limitation, persons are allowed to be expert in certain issues and areas.  However, it is expected, if not required, for the person with an expertise in a certain area to have a comprehensive and holistic knowledge of the fundamentals in the management of patients with a breast health or wellness problem.  He / she must know at least the goals and objectives in the management  of patients with a breast health or wellness problem.  He / she must know the processes involved to achieve the goals and objectives.  His  / her main contribution should be to align and integrate his / her expertise into the fundamental framework of the management of a patient with breast health or wellness problem.

“Person”

The  person can be anybody.  He / she may be a physician, a nurse, or any other health care professional.  The physician can be of any type or any specialty  – a general surgeon, a surgical oncologist, a medical oncologist, a radiation oncologist, an general medical practitioner, a paediatrician, a gynaecologist, a pathologist, a radiologist, etc.

The person can be anybody as long as he / she is constantly interested and devoting a big portion of his /her time, lifetime, weekly if not daily, in studying and researching to develop and refine his / her expertise, in terms of knowledge and skills, in the management of certain, if not all issues, related to breast health or wellness.

All the so-called breast specialists must work in a transdisciplinary manner for the sake of patients with a breast health or wellness concern.  Nobody can claim he/she knows and can do everything.

All the so-called breast specialists working in a transdisciplinary manner must use the same holistic breast wellness management framework  as a guide in managing patients with a breast health or wellness concern so that there will be coordination, alignment and integration among them, again for the sake of the patients.

All the so-called breast specialists working in a transdisciplinary manner must have a physician-orchestrator to facilitate achievement of coordination, alignment and integration in patient management.  He /she is usually a clinician, one who is an expert in physical examination of the breast and who has direct and long interaction with patients.  Furthermore, the physician-orchestrator is someone among the breast specialists in the transdisciplinary team who is known to have acquired the most extensive comprehensive and holistic knowledge and expertise in the fundamentals in the management of patients with a breast health or wellness problem as a result of training and experience.
